Step 1
Web Addresses: Review all company websites to ensure the main page contains the following: • Applicant’s legal name, even if a trade name is listed on the website • Applicant’s NMLS number • A link to NMLS Consumer Access, labeled as "NMLS Consumer Access" Enter all company websites in Web Addresses, including those used to solicit customers.Submitted via: NMLS

Qualifying Individual: Enter the name of your company’s Designated Broker in the Qualifying Individual section and select Washington.Submitted via: NMLS

MLO Sponsorship: Submit a sponsorship in NMLS for an individual who holds or is applying for a WA MLO License.Submitted via: NMLS

Organizational Chart: Upload an organizational (ownership) chart showing the company's ownership.Submitted via: Upload under the Document Type "Organizational Chart/Description"
Step 6
Management Chart: Upload a management chart showing the applicant's divisions, officers and managers. At minimum the chart must include all individuals listed in the Direct Owner/Executive Officers section.Submitted via: Upload under the Document Type "Management Chart"
Step 7
Business Plan: Upload your company’s business plan which at minimum should outline the products/services to be offered, target markets, and marketing strategy.Submitted via: Upload under the Document Type "Business Plan"

Trust Accounting: Review RCW 19.146.050 and WAC 208-660-410 before providing one of these forms: • Use the “Certificate of Compliance and Authorization to Examine Trust Accounts” form to report each trust account used with WA loans. The trust account(s) must be located at a federally insured depository institution in Washington. You complete the top portion of the form, have the bank complete the bottom portion and the bank notarizes it. • Use the “Alternative Certificate of Compliance” form if you intend never to accept monies on behalf of borrowers for the payment of third party service providers.Submitted via: Upload under the Document Type "Trust Account Authorization"

Activity Declaration: Upload the Activity Declaration form completed by an individual listed in the Direct Owner/Executive Officer section. Mortgage Broker Activity DeclarationSubmitted via: Upload under the Document Type "Additional Requirements"
Step 11
Designated Broker: The individual appointed to serve as the company's Designated Broker must complete all of the following items: • Apply for the Designated Broker Registration through NMLS. In that filing, upload the following documents: • W2s, 1099s, or signed corporate tax returns to support two years of mortgage-related experience. These documents must be uploaded in the Verification of Experience section of the Designated Broker's individual form. • A PDF document with the Designated Broker's name, NMLS # and the year the Designated Broker received their high school diploma or equivalent. The document must be uploaded in the Vertification of Experience section of the Designated Broker's individual form. • A PDF of the Desiganted Broker's test passage certificate for the Washington Desiganted Broker test. The passage certificate was provided at the PearsonVue testing center following passage of the test. The document should be uploaded under the document type Verification of Experience on the Designated Broker's individual form. • The company submits a sponsorship request for the Designated Broker Registration.Submitted via: NMLS

Other Trade Name: Enter the name(s) your company uses to conduct bsuiness, other than its legal name, in the Other Trade Names section. you cannot use a trade name which is similar to another existing licensee's name.Submitted via: NMLS

Resident/Registered Agent: Enter your company's Registered Agent informaiton as reported to the Washington Secretary of State.Submitted via: NMLS

Disclosure Questions & Explanations: Enter an explanation for any 'yes" answer to the disclosure questions. Enter a separate explanation for each event. Upload all supporting documents, in PDF form, for each event.Submitted via: NMLS

Certificate of Authority (LLC or Corporations Only): Upload your company’s Certificate of Authority issued by the Washington Secretary of State’s Office.Submitted via: Upload under the Document Type "Certificate of Authority/Good Standing Certificate"
