You may use this application ONLY IF YOU MEET ALL OF THE FOLLOWING QUALIFICATIONS: • You are an active service member or the spouse of an active service member; • You have relocated to Maryland because of orders for service; • You hold active mortgage loan originator license in at least one other state; • All mortgage loan originator licenses you hold are in good standing, with no disciplinary actions taken or pending; • You have met the pre-licensing education requirements for a Maryland mortgage loan originator license; and • You wish to originate Maryland mortgage loans. If qualified, you may originate Maryland mortgage loans under this application in lieu of a Maryland mortgage loan originator license.

    Company Sponsorship: A sponsorship request must be submitted by your employer. MD will review and accept or reject the sponsorship request.

    Employment History: The business address listed in the Employment section of the Individual Form (MU4) must match the address of the registered location in the Company Relationship.

    Military Orders: Upload a copy of your, or your spouse's, orders showing that you currently reside in Maryland due to such orders.
  4. Step 4

    Marriage Certificate: If you are the spouse of an active service member, provide a copy of your marriage certificate.
  5. Step 5

    Affidavit: Provide a notarized affidavit affirming that: • You are the individual described in this filing; • All statements made in this filing are true, accurate, and complete; • You understand the requirements for a Maryland mortgage originator license and the activities permitted thereunder; • You meet all requirements for a Maryland mortgage loan originator license and will comply with those requirements and the scope of activities permitted thereunder; • You are in good standing in all states in which you hold a mortgage loan originator license.
