Step 1
Other Trade Name: If operating under a name that is different from the applicant’s legal name, that name (“Trade Name”, “Assumed Name” or “DBA”) must be listed under the Other Trade Names section of the Branch Form (MU3) and the Company Form (MU1). If operating under an “Other Trade Name”, upload Ohio Secretary of State Trade Name Certificate of Registration regarding ability to do business under that trade name. This document should be named OH-RM-Trade Name-Assumed NameSubmitted via: NMLS and Upload in NMLS: under the Document Type Trade Name/Assumed Name Registration Certificates in the Document Uploads section of the Company Form (MU1).
Step 2
Deed/Lease/Sublease: Upload a copy of the Branch Office Deed and/or Lease/Sublease. If a residence, upload in NMLS proof that business can be conducted at that location (Lease and/or Branch Written Agreement), including zoning approval letter and proof of no deed restrictions. This document should be named RMLA Branch Location DocumentsSubmitted via: Upload in NMLS: under the Document Type Branch Written Agreement in the Document Uploads section of the Branch Form (MU3).
Step 3
Branch Manager: A Branch Manager must be designated for each registered location. A Branch Manager is defined as an individual whose principal office is physically located in, who is in charge of, and who is responsible for the business operations of a branch office. For purposes of Ohio, this individual may be the “Qualifying Individual” on Company Form (MU1), also known as the “operations manager” under Ohio law. NOTE: If the branch manager is not an approved Ohio MLO, also list an approved Ohio MLO associated with the branch location. Upload the listed branch manager and approved Ohio MLO (if applicable) to the MU2 in the NMLS.Submitted via: NMLS
General information
Who Is Required to Have this License
This branch registration is required for each location to be maintained by the holder of a Residential Mortgage Loan Act (“RM”) Registration, other than the main office identified on the MU1 filing. Please refer to Ohio Revised Code Chapter 1322 for more information.

Pre-Requisites for License Applicaitons
• This branch license should only be applied for by a company that holds or is seeking to obtain the Residential Mortgage Lending Act registration.
