Step 1
SC-DCA and NMLS Fees: See fees chart in Resource Center. Credit Report for Control Persons: $15 per control person. Submission of Branch Form (MU3): Complete and submit the Branch Form (MU3) in NMLS. This form serves as the application for the license through NMLS.Submitted via: NMLS (Filing submission)
Step 2
Other Trade Name If this branch is operating under a name that is different from the applicant’s legal name, that name (“Trade Name”, “Assumed Name” or “DBA”) must be listed under the Other Trade Names section of the Company Form (MU1). The South Carolina Department of Consumer Affairs does not allow more than one “other trade name” to be used in South Carolina.Submitted via: NMLS
Step 3
Branch Written Agreement: Submit a copy of the agreement between the licensed company and Branch Manager, in compliance with applicable state and federal law. The document must include the following information: • Location of Branch • Must be signed by the branch manager and licensed company representative. This document should be named [Branch NMLS ID#] Branch Manager Agreement.Submitted via: NMLS Upload in NMLS under the Document Type Branch Written Agreement in the Document Uploads section of the Branch Form (MU3).

South Carolina Mortgage Broker Fee Agreement: Every licensed branch location must have and use a South Carolina Mortgage Broker Fee Agreement. Click here to access form on Department’s website. Must include company name and branch address on Fee Agreement. This document should be named [Branch NMLS ID#] SC Fee AgreementSubmitted via: NMLS Upload in NMLS under the Document Type Document Samples in the Document Uploads section of the Branch Form (MU3).
Step 5
Verification of Experience: If branch manager’s MU4 Employment History does not clearly establish required experience, provide a license-specific resume with detailed job descriptions and/or duties performed evidencing three years' experience in financial services. Detailed job descriptions and duties with all employers need to be incorporated into a resume to demonstrate experience related to financial services. This document should be named [Document Type] – License Name.Submitted via: NMLS Upload in NMLS under the Document Type Verification of Experience in the Document Uploads section of the Individual Form (MU2).
Step 6
Branch Manager: A Branch Manager must be designated for each licensed location. A Branch Manager is defined as an individual whose principal office is physically located in, who is in charge of, and who is responsible for the business operations of a branch office. • Branch Manager must hold an active SC-DCA MLO or QMLO license. • Branch Manager must satisfy the statutory experience requirements: three years of experience in financial services. Each location licensed by SC-DCA must have a unique branch manager. The Branch Manager must not be the qualifying individual or a branch manager of another location. Branch Manager (MU2) Attestation: Complete the Individual Form (MU2) in NMLS. This form must be attested to by the applicable branch manager before it is able to be submitted along with the Branch Form (MU3). Credit Report: Branch Managers required to authorize a credit report through NMLS. Individuals will be required to complete an Identity Verification Process (IDV) along with an individual attestation before a license request for your company can be filed through NMLS. This authorization is made when the Individual Form (MU2) is submitted as part of the Branch Form (MU3).Submitted via: NMLS
