Step 1
Credit Report: See the License Requirements and Fees Chart on the NMLS Resource Center for details related to this Requirement. Branch Managers are required to authorize a credit report through NMLS. The Branch Manager will be required to complete an Identity Verification Process (IDV) along with an individual attestation before a license request can be filed through NMLS.Submitted via: NMLS

Lease or Deed: Upload lease or deed for the branch location directly to NMLS.Submitted via: NMLS
Step 3
Planning and Zoning: Upload approval from local Planning and Zoning board for in-home business. Required for residential locations only.Submitted via: NMLS
Step 4
Disclosure of Residence: Upload KY Disclosure of Residence Form. Required for residential locations only.Submitted via: NMLS
Step 5
Prior Activity Attestation: Upload a signed statement indicating if the applicant has ever conducted Kentucky mortgage activity at this location. If transactions have occurred, please provide the total number and dollar amount of transactions for each year. Attestation must be signed by a control person.Submitted via: NMLS
Step 6
Branch Manager: A branch manager must be designated for each licensed location at all times. Branch manager is defined as an individual whose principal office is physically located in, who is in charge of, and who is responsible for the business operations of a branch office.Submitted via: NMLS

Branch Authorization Form: Upload Kentucky Branch Authorization form directly to NMLS.Submitted via: NMLS
Step 8
Sponsored Mortgage Loan Originator: Each location must assign and sponsor at least one MLO.Submitted via: NMLS
